Oral Health and Public Health Systems in Europe

Image
Online Journal of Dentistry & Oral Health  Oral health care in Europe is provided by a diverse range of services. In the Nordic countries children are eligible for public dental services, whereas adults and the majority of older people receive health care from private dental practitioners with national and local co-payment systems [1]. In southern Europe, private dental practitioners dominate, and third-party payment is based on private insurance or costs are paid by employers [2]. While in eastern Europe and central Asia, oral health services are based on a blend of public and private systems [3,4]. Digital Smile Design, Guided Surgery, Delivering Facial Driven Implant Treatment in A Failed Dentition: Case Reports

Image
Online Journal of Dentistry & Oral Health    The Digital Smile Design (DSD) digital implant treatment planning solutions allow for predictable prosthetic results, DSD helps in visualizing the end product through a blue print. The transition of patients from failing dentition to complete-arch implant rehabilitation often means that the patient is rendered edentulous and has to wear a removable complete denture for a period of time. Many patients find this unacceptable. With the immediate loading concept, it is possible to offer our patients a third dentition on the day of extractions. Today, many people suffering failed dentition, moving towards edentulism, getting back their smile with a fixed prosthesis could be priceless. This treatment provides a fixed interim prosthesis use throughout the rehabilitation process, allowing patient comfort and prosthodontic control. Influence of Shoulder and Chamfer Margin Design on the Stress Distribution Pattern in Different All Ceramic Restorations by Three-Dimensional Finite Element Analysis

Image
Online Journal of Dentistry & Oral Health  The purpose of this study was to evaluate the influence of shoulder and chamfer margin design on stress distribution pattern in different all ceramic restorations. Six three-dimensional finite element models representing mandibular second premolar tooth were constructed, using a Finite Element Analysis Software (ANSYS version 14; ANSYS Inc., Canonsburg, PA, USA). Three models each with shoulder and chamfer finish line were created after tooth preparation by reverse engineering, using Pro E software. Each model with shoulder finish line margin had Bruxzir (Glidewell labs), Lava (3M ESPEE) and IPS e.max Press (Ivoclar Vivadent) all ceramic restorations fabricated over them. Same was done with the models having chamfer finish margin. A distributed load of 600 N was applied on the restorations to analyze the stress distribution pattern in different all ceramic restorations having different marginal geometry. The stress distribu...

Image
Online Journal of Dentistry & Oral Health Opportunistic oral fungal infections have spread, especially in denture wearers. Denture stomatitis (DS) is a common inflammatory reaction with multifactorial etiology. It is usually associated with Candida species, particularly Candida albicans, due to its high virulence, ability to adhere and form biofilms on oral cavity tissues and denture surfaces. This study highlights the risk factors of Candida albicans-associated denture stomatitis. The study was carried out at Prosthodontics Departments at the Faculty of Dentistry- Sana’a University, Sana’a city - Yemen. Investigation of Maturation Failure in Revascularization

Image
Online Journal of Dentistry & Oral Health  One of the major aims of revascularization therapy is to complete the physiological mechanism of the tooth. It is very difficult to regenerate the pulp which consists of a specialized tissue and maintain its durability especially in a previously infected area. The purpose of this review is to investigate the reasons for the uncompleted maturation stage in the regenerating pulp tissue. Hepatitis B Virus among Dental Clinic Workers and the Risk Factors Contributing for its Infection

Image
Online Journal of Dentistry & Oral Health The study included 246 dentists and 263 dental assistants, the sero-prevalence of current hepatitis B virus infection was 6.1%; while prevalence of life time exposure to hepatitis B virus infection was 18%. Prevalence of needle stick injuries, exposure to skin and to mucous membranes were 45.6%, 26.5% and 25.3% respectively. Cuts were also common with 41.1% of participants reporting a cut in a period of one year preceding the survey. Regular use of gloves, face mask, and glasses were reported by 74.4%, 28.3% and 25.3% of participants respectively. Only 30.1% of respondents were vaccinated against hepatitis B virus infection and 52.1% were susceptible and could potentially be protected through vaccination. Longer duration in service was associated with risk of infection (OR ranged from 2.2-2.65; P value = 0.002). Being assistant (OR = 1.97; P value = 0.001) was associated with a higher risk of hepatitis B virus infection. For More Open...

Mineral Trioxide Aggregate (MTA) vs Calcium Hydroxide in Direct Pulp Capping – Literature Review

Image
Online Journal of Dentistry & Oral Health  Direct pulp-capping is a treatment for exposed vital pulp involving the placement of a dental material over the exposed area to facilitate both the formation of protective barrier and the maintenance of vital pulp. Direct pulp capping has been used as an alternative approach to the maintenance of vital pulp so that many tooth extractions and root canal treatments could have been avoided through the conservative approach of direct pulp capping. For this purpose different kinds of materials used such as Zinc Oxide Eugenol, Glass Ionomer (GI), Resin Modified Glass Ionomer (RMGI), Adhesive systems, Calcium Hydroxide, Mineral Trioxide Aggregate (MTA) and Bio dentine. The MTA clinically performed more effective than conventional Calcium Hydroxide liner as a direct pulp capping material, showed higher success rate than dycal. MTA easier to use clinically as a direct pulp capping material. MTA provided better long term results more effec...

Individual Preferences on Grading Systems in Dental Schools

Image
Online Journal of Dentistry & Oral Health   The objective of this study was to determine the type of grading system (pass/fail, pass/fail/pass with honors, traditional A-F grading) that individuals prefer in a sample of U.S. dental schools. Fifteen U.S. dental schools were selected, six of which used some type of pass/fail grading, the remainder used traditional grading and were randomly selected from a list of all U.S. dental schools. Six schools of the fifteen received the questionnaire which included demographic questions, questions on status as student or faculty, questions on current school grading system and individual preference for grading system types. Data was analyzed using an ANOVA with post hoc analysis using the Bonferroni method. Faculty significantly strongly disagree with pass/fail grading as compared to students (p=0.001, CI 3.07-3.70). Additionally dental school faculty were more likely to disagree with the pass/fail/pass with honors grading system th...

A Simplified Approach to Rehabilitation of Swallowing: Labiotherapy

Image
Online Journal of Dentistry & Oral Health Functional rehabilitation is now part of the treatment of dysmorphoses requiring orthodontic treatment. But it is actually all practitioners who should be concerned with acquiring a myofunctional balance in the oral zone. Joint prostheses, implants, complete prostheses, periodontal health all these daily acts absorb muscle forces generated by oral praxies. Abandoned during the initial training, these functional rehabilitation procedures, which are time-consuming and maltreated by the nomenclature, are insufficiently integrated into the daily practice of general practitioners . The contribution of functional educators [1] to wear all night in addition to a series of exercises, has allowed the gradual integration of this approach in the treatment plans for complete rehabilitation of the masticatory system, without the need the help of speech therapists also facing the difficulty of re-education of swallowing. Association Between the Salivary Streptococcus Mutans Levels and Dental Caries Experience in Adult Females

Image
Online Journal of Dentistry & Oral Health  Streptococcus mutans  has been concerned like one of the main etiology of dental caries [1,2]. It is known that tooth surfaces inhabited with  S. mutans  are on a high risk of developing caries [1- 3]. Appearing in people with a relatively high caries experience, a positive relationship between high salivary levels of Streptococcus mutans and dental caries experience have been reported [4,5]. Those with high levels of  Streptococcus mutans  as well acquire more coronal and root caries in permanent restorations and temporary restorations than persons appearing in the same population with lower intensity of  Streptococcus mutans  [5-7].  S. mutans  salivary levels are straight linked to the number of tooth sites colonized [8] and to their amount in dental plaque [9]. Use of Endodontic Cores for Rigid Screw Retention of the Natural Dentition to Dental Implants by Means of Fixed Partial Prostheses for the Atrophic Mandible: Long-Term Clinical Results

Image
Online Journal of Dentistry & Oral Health Implant-supported prostheses have been first choice for treating lost teeth. However, there are challenging cases that only reach resolution by exploring unconventional options. In order to overcome specific shortcomings, tooth-implant supported prostheses have shown to be a predictable treatment option. The most significant difference between an implant-supported prosthesis, when compared to a tooth-supported, is the absence of a periodontal ligament (PDL). This anatomic structure is responsible for tooth movement, shock absorption, and proprioception [1- 3]. Implant-supported prostheses have the same limitations and challenges as the tooth-supported prostheses [4,5], which do not have the benefit of a PDL. Therefore, splinting an implant to a tooth may indirectly increase the proprioception of the implant and, consequently, protect the stomatognathic system.
